Telehealth Informed Consent

This Telehealth Informed Consent ("Consent") is entered into between you ("Patient," "you," or "your") and Nova Wellness LLC, trading as Nova Health RX ("Nova Health RX," "we," "our," or "us").

By using our website, completing a medical intake questionnaire, scheduling a consultation, or receiving healthcare services through Nova Health RX, you acknowledge that you have read, understood, and agree to this Telehealth Informed Consent.

What is Telehealth?

Telehealth is the delivery of healthcare services using electronic communications, including secure online platforms, video conferencing, telephone consultations, messaging, and other digital technologies.

Telehealth allows licensed healthcare providers to evaluate, diagnose, prescribe medications where appropriate, monitor treatment, and provide follow-up care without requiring an in-person office visit.

Healthcare services provided through Nova Health RX are delivered by independent licensed medical providers practicing within their licensed jurisdictions.

Nature of Services

Through Nova Health RX, you may receive services including but not limited to:

  • Medical evaluations

  • Review of your medical history

  • Treatment recommendations

  • Prescription of medications when medically appropriate

  • Ongoing treatment management

  • Follow-up consultations

  • Lifestyle and wellness recommendations

Not every patient is an appropriate candidate for telehealth treatment.

Healthcare providers may determine that:

  • Telehealth is not appropriate for your condition;

  • Additional information is required;

  • An in-person medical examination is necessary; or

  • Treatment cannot safely be provided remotely.

No Guarantee of Treatment

Submission of your medical information or payment does not guarantee:

  • a diagnosis;

  • a prescription;

  • continued treatment;

  • medication approval; or

  • any specific medical outcome.

Treatment decisions are made solely by the licensed medical provider based upon their independent medical judgment.

Risks of Telehealth

As with any healthcare service, telehealth has limitations and risks, including:

  • technical interruptions;

  • internet or communication failures;

  • incomplete medical information;

  • inability to perform a physical examination;

  • delays caused by technology failures;

  • privacy risks despite reasonable safeguards.

Although Nova Health RX uses commercially reasonable security measures, no electronic transmission or storage system can be guaranteed to be completely secure.

Benefits of Telehealth

Potential benefits include:

  • convenient access to healthcare;

  • reduced travel time;

  • improved continuity of care;

  • increased accessibility to licensed providers;

  • timely follow-up appointments;

  • secure communication with your healthcare provider.

Patient Responsibilities

You agree that you will:

  • provide complete, truthful, and accurate medical information;

  • update your information if your health changes;

  • disclose all medications, supplements, and allergies;

  • follow treatment instructions provided by your healthcare provider;

  • attend recommended follow-up appointments;

  • immediately seek emergency care when appropriate.

Providing inaccurate or incomplete medical information may affect your diagnosis and treatment.

Emergency Situations

Nova Health RX does not provide emergency medical services.

If you believe you are experiencing a medical emergency, immediately:

  • Call 911

  • Go to your nearest emergency department

  • Contact your local emergency services

Do not rely on telehealth messaging or online communications during an emergency.

Prescriptions

Healthcare providers may prescribe medications when medically appropriate and permitted under applicable law.

Prescription decisions are made solely by the treating licensed medical provider.

Some medications:

  • may require laboratory testing;

  • may require ongoing monitoring;

  • may require an in-person evaluation;

  • may not be prescribed through telehealth.

Nova Health RX cannot guarantee that any medication will be prescribed.

Medication Availability

Medication availability may vary based upon:

  • state regulations;

  • pharmacy inventory;

  • provider discretion;

  • medical appropriateness.

Nova Health RX cannot guarantee that every medication will always be available.

Privacy

Your health information is handled in accordance with our Privacy Policy and applicable federal and state privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA), where applicable.

Electronic communications are encrypted using commercially reasonable safeguards.

Communications

You authorize Nova Health RX and its affiliated healthcare providers to communicate with you electronically, including by:

  • email;

  • secure messaging;

  • SMS/text messages;

  • telephone.

Standard messaging and data rates from your mobile carrier may apply.

Electronic communications may include:

  • appointment reminders;

  • treatment information;

  • prescription updates;

  • billing notifications;

  • follow-up requests.

Right to Withdraw Consent

You may withdraw your consent to receive telehealth services at any time by contacting Nova Health RX.

Withdrawal of consent will not affect healthcare services already provided but may prevent future telehealth treatment.

Medical Records

Records created during your telehealth treatment become part of your confidential medical record and will be maintained in accordance with applicable legal and regulatory requirements.

By using Nova Health RX, you acknowledge and agree that:

  • You understand the potential risks and benefits.

  • You understand that telehealth has limitations compared to in-person care.

  • You understand that no diagnosis, prescription, or treatment is guaranteed.

  • You have had the opportunity to ask questions.

  • You consent to receive healthcare services through telehealth.

  • You understand that healthcare decisions are made solely by the treating licensed medical provider.

  • You certify that the information you provide is accurate and complete to the best of your knowledge.

Electronic Consent

By clicking "I Agree," "Continue," "Submit," or by using the Nova Health RX platform, you acknowledge that your electronic signature is legally binding and constitutes your informed consent to receive healthcare services through telehealth.

If you do not agree to this Telehealth Informed Consent, you should not use Nova Health RX's services.
