This Telehealth Informed Consent ("Consent") is entered into between you ("Patient," "you," or "your") and Nova Wellness LLC, trading as Nova Health RX ("Nova Health RX," "we," "our," or "us").
By using our website, completing a medical intake questionnaire, scheduling a consultation, or receiving healthcare services through Nova Health RX, you acknowledge that you have read, understood, and agree to this Telehealth Informed Consent.
What is Telehealth?
Telehealth is the delivery of healthcare services using electronic communications, including secure online platforms, video conferencing, telephone consultations, messaging, and other digital technologies.

Treatment cannot safely be provided remotely.
No Guarantee of Treatment
Submission of your medical information or payment does not guarantee:

a prescription;

continued treatment;
-
medication approval; or
-
any specific medical outcome.
Treatment decisions are made solely by the licensed medical provider based upon their independent medical judgment.
Risks of Telehealth
As with any healthcare service, telehealth has limitations and risks, including:

Electronic Consent
